Co-pays are due at the time of your visit to the office.
We will ask to see your insurance card (or your child’s card if your company issues one in your child’s name) so that we can verify that we have the correct information. Please remember to bring your card or your child’s card to the office each time that you come in. If you change insurance companies, you must notify us by sending a copy of the front and back of the card to our office.
We charge a fee if you miss a scheduled visit for a physical exam or if you do not cancel at least 1 business day in advance.
We only send you a monthly statement if you have a balance due. Usually, this means that your insurance company has transferred the responsibility to you for either our entire charges or for part of the charges. If you do not agree with the transfer of charges to you by your insurance company, please call your company right away to straighten out the situation.
